Dayton Delivers is the City of Dayton's customer services program, featuring an online app, call centers and a wide variety of activities occurring in neighborhoods and locations throughout the community.
Survey: Dayton residents may soon receive a phone call from a Wright State University survey team representing the City of Dayton asking residents about the City's customer services. Please take time to respond; your survey answers can provide valuable feedback that will help the City improve the quality and delivery of essential services.
If you don't receive a survey phone call, you can still provide input on City of Dayton customer services by completing the online version of the survey.

OFFICIAL RULES for Drawing
All participants, by virtue of their entry, agree to be bound by the following terms and conditions.
Sponsors. This contest is brought to you by the City of Dayton (“City”). Contest sponsor can be reached by mail at: Dayton City Hall, Attn: Public Affairs, 101 W. Third Street, Dayton, Ohio 45402.
Eligible Participants. Participants must be a legal U.S. and Ohio resident of at least 18 years of age or older and must participate in a phone survey. City elected officials, officers, employees, and their immediate families or persons living in their households are not eligible to enter or win.
One prize per household. One winner will be selected at random from the entries on September 10, 2015. Winners will be notified within twenty-four (24) hours of the prize drawing phone or email. The City is not responsible for malfunctions or errors in submitting information for entry.
Revocation of prize. If a winner is not able to claim his or her prize within one week of notification, or if the winner fails to provide proper identification, or if an announced winner is determined to be an ineligible participant, then in any such case the City shall declare the prize forfeited, and proceed to choose a secondary winner.
Odds of Winning. Two prizes will be awarded. Each entrant’s odds of winning depend on the number of entries received. The number of entries is limited to no more than the first 500 survey participants.
Privacy. The City will not sell, trade or otherwise circulate information provided by participants to third parties. Information provided during entry will be kept solely for the City’s record. The City reserves the right to include participant emails in its email listserv to send future official City updates. Participants who wish to unsubscribe from such email updates can do so by clicking “Unsubscribe” at the bottom of any email from the City.
Release of Liability. Entrants agree that the City shall not be responsible or liable for any losses, damages or injuries of any kind resulting from participation in the drawing or from the possession and/or use or misuse of the prize(s), and have not made any warranty, representation or guarantee express or implied, in fact or in law, with respect to the prize, including, without limitation, to such prize’s quality or fitness for a particular purpose.
Decisions are final. The decisions of the City concerning any and all matters with respect to this drawing shall be final.
Reservation of rights. The City reserves the final judgment on all matters pertaining to this drawing including the right to change or modify the rules or prizes, or to discontinue without prior notice.
Obtaining Rules and List of Winners. These official rules for the Survey Drawing are available at www.daytonohio.gov. A copy of these official rules can also be obtained by writing the City of Dayton, and enclosing a self-addressed stamped envelope to: Dayton City Hall, Attn: Public Affairs, 101 W. Third Street, Dayton, Ohio 45402. A copy of a list of winners of the contest can be requested at the same address following the end of the contest. Requests received more than three months after the end of the contest will not be honored.
Responsibility for taxes and other costs associated with winning. The approximate retail value is $400.00. Winners shall assume any and all responsibility as to federal, state, or local taxes due in connection with the prize. Winners will also assume any and all additional costs in connection with using prizes as awarded.
Applicable law. This contest and corresponding rules are governed by Ohio law, and any dispute or litigation arising from this contest must be brought in a court of competent jurisdiction in Montgomery County, Ohio. This contest is subject to all applicable federal, state, and local laws. Participants hold all responsibility to comply with any applicable federal, state or local laws in connection with their entry.
Publicity. The prize winner grants the City full and unlimited permission to use the prize winner’s personality, including the prize winner’s name, likenesses, and creative entries for promotion and publicity purposes, including all advertising, promotions and features without further compensation.
Miscellaneous. Void where prohibited. No purchase necessary to enter or win.
